About Family Care Connections

Family Care Connections is dedicated to improving the lives of individuals with special needs by providing a range of services, including respite, habilitation, attendant care, adult day programs, group homes, and employment training. Our compassionate and cohesive approach ensures that every need is addressed, and we are proud to be funded through the Division of Developmental Disabilities. All of our clients meet DDD Title 19 criteria and have been diagnosed with autism, epilepsy, cerebral palsy, and/or an intellectual disability. We serve all of Phoenix, including the west valley areas of Glendale, Peoria, Avondale, Buckeye, El Mirage, Goodyear, Litchfield, and Surprise. Day Programs

Family Care Connections seeks to meet the temporal, physical, social, and emotional needs of its members. We have Day Treatment and Training facility to meet these needs!

Respite, Habilitation & Attendant Care

HCBS services address the needs of the individual with functional limitations with everyday activities, like getting dressed or bathing however, they can also be used to help give the main caregiver a break.

Employment Training

FCC Employment Services supports the member’s purpose through job training skills. Members will have opportunities to build their job training skills through hands-on classroom and community learning experiences.

Group Homes

We currently have licensed residential group homes in the West Valley Area. We offer comfortable, family-oriented homes with up to 3-4 people per home. All homes have 24-hour, 7-day-a-week quality care and provide a safe and healthy environment.
